How to make a claim
Ensure you have the following evidence:
- A brief letter explaining the circumstances of the death
- A certified copy of the original Death Certificate
- Copies of any additional information that supports the claim
Collate your evidence and send us your claim by:
- Secure document upload
- Post to PO Box 108022 Symonds St, Auckland
- Fax to 0800 282 646
- a letter containing the policy number or personal loan or credit card account number and a brief explanation of the circumstances of the death; and
- certified copy of the original Death Certificate.
- Additional information or documentation that supports the claim such as a police report, medical reports, Coroners report or newspaper clippings if relevant.

- Have your doctor or medical specialist complete the section headed ‘Section C: Medical Certificate’ on the claim form;

Call us and make a claim on

- complete the Price Protection claim form;
- have completed the Price Verification section of the claim form (if the item is reduced by more than $200 and you do not have the original catalogue, advertisement or promotional flyer (with a printed visible date) from the store of purchase showing the new reduced price);
- provide proof of original purchase price; and
- sign and date the declaration (or agree to the declaration if using the online claim form).
